Memorable journey is an approach to Alzheimer’s Disease and dementia care that is both patient-centered and family-focused. With the whole person in mind, care is both individualized for patients and evidence-based.
Heart Touch Journey
Heart Touch Journey is a patient and family centered clinical specialty program designed to help cardiac disease patients stay in their home setting and reduce uncomfortable and lengthy trips to the emergency room.
Clear Journey
Clear Journey offers education, support and treatment through a team approach to addressing the medical, emotional, and spiritual needs of pulmonary patients, their caregivers, and their families.
